REGISTRATION INFORMATION

Interested candidates should complete the application form . The completed applications must be received no later than To Be Announced. Since space in the Workshop is limited and admission is competitive, earlier application is strongly recommended.

The application form is available in both Word.doc and Adobe.pdf formats. The .doc form can be filled out with Word and emailed as an attachment. The .pdf form must be printed and scanned before emailing. Or, either form can be printed and then faxed or sent via regular mail to:
